Noise exposure hazard at the Dr. Soetomo Hospital, Surabaya Indonesia
Noise exposure in a hospital is known to have considerable impact not only to the healthy workers, but the ability for speedy recovery to the patients as well. However, recent investigations showed that the noise emission in a hospital tends to increase from year to year, for example John Hopkins Hospital (Busch-Vishniac, et. al., 2005). Dr. Soetomo Hospital is the largest hospital in eastern part of Indonesia serving 2,500 outpatients and 1,500 inpatients per day as of January 2017, tripled from 2014. This also implies that the activities also increase significantly, from supporting services such as laundry to clinical services.
In this paper, we present our recent study on the noise emission focusing on the supporting activities of the hospital particularly water pump station, boiler, central kitchen, incinerator and workshop. The aim is twofold, namely to investigate the noise dose exposure to the workers of each supporting facility and to map the noise emission to the surrounding.
We measured the noise emission level of the facility during its respective working hours. We set up calibrated sound level meters (SLM) at about 1.25m above the ground, placed them at several free-field measurement points. The data acquisition at the water pump station and the boiler were collected at 12 data per minute for 48 hours continuously. The central kitchen, the incinerator and the workshop were only measured at their respective working hours for two consecutive days. We also placed the SLMs outside the rooms as well, to investigate the noise level reduction with respect to distance from the source and attenuation.
The results showed that all the facilities emitted higher level (in decibel) than allowed by the WHO. For the continuous noise, the average noise level for the water pump station was about 103 dB SPL (sound pressure level) and the workers were exposed almost 100 times more than that of the allowed noise exposure daily dose. The crushers at the incinerator facility were the highest of all intermittent noise sources at 112 dB SPL which is equivalent to 110 times higher noise exposure. Furthermore, the noise level difference between outside and inside the water pump station was only reduced 1 dB, which suggests that either the sound enclosures was no longer effective or most of the noise energy lied between 60-200 Hz (lower frequency band). Although the noise dose exposed to the workers were considerably exceeding the WHO safety level, the workers were still reluctant to wear any ear protections.
To summarize, the overall results showed that either the continuous or intermittent noise source the workers were exposed to high dose of noise daily. From the distribution noise level contour map, it was shown that firstly, the role of the sound insulation should be investigated more thoroughly and secondly, the noise sources at the Dr. Soetomo Hospital may have affected its respective neighboring rooms. In the on-going investigations, we are classifying the noise characteristics in term of dominant frequency bands to mitigate the noise emission efficiently.

Hearing has an important role for the social life of human being. In order to provide a good quality of communication with others, required the ability to hear, listen and understand the speech properly since the early age. The Hearing & Communication Center ( HCC) in Dr. Soetomo Hospital - Faculty of Medicine Airlangga University was established. HCC is placed in Audiology-Neurotology Clinic, ENT Head & Neck Surgery Department - Dr. Soetomo Hospital and Faculty of Medicine Airlangga University. The joint work with reguler visit of professionals from Western Australia has been existing until now. This work is possible under the program between two cities, Surabaya - Perth as sister cities / sister province. The cooperation includes an exchange programs in the field of culture, research and health services (Telemedicine and Audiology). HCC in Surabaya already be a member of Hearing International organization. One of The recent project that is developed under cooperation with Government and NGO in Perth with the Provincial Government of East Java, through the commitment of regency in Sidoarjo, Gresik, Surabaya. The pilot project of Resource Center (RC) is finally take placed in five centers. There are five Hearing and Communication Center (HCC), established in Gresik, Sidoarjo, Surabaya ( Unair hospital), Karya Mulia School of the Deaf and Social Rehabilitation Unit for Disabilities of Deaf & Mute in Bangil-Pasuruan. The next program is to support student exchange and research. Collaboration work between Faculty of Medicine Universitas Airlangga and School of Audiology University of Western Australia to allow the activities. It will support an easy access for hearing services, implementing early detection and intervention for Congenital Deafness, developing inclusive school, and habilitation / rehabilitation program for the Deaf and hearing impaired in East Java province
